Terms and Conditions

Children’s Hospital and Health System, Inc. (Children’s) is pleased to offer MyChart to its patients, subject to compliance with these Terms and Conditions. Please review the following information carefully. The Terms and Conditions outline Children’s and your responsibilities related to using MyChart. Your use of MyChart is your agreement to abide by these Terms and Conditions. If you do not agree to these Terms and Conditions then exit MyChart now.

1. What is MyChart? MyChart is a web-based computer system that allows you to view portions of your health care record, securely message your providers, and schedule appointments electronically.

2. Login and Password- You agree not to share your login name and password with others. You agree to change your password if someone else discovers it. You may be required to change your password on a regular basis.

3. Eligibility- Parents/legal guardians can create and access a MyChart account for their minor child up to age 14. Teenage patients (age 14-17), with the consent of their parent/legal guardian, can create and access their own MyChart account. Parents/legal guardians, with the consent of their teenage child, can create and access their teenage child’s MyChart account. Adult patients (age 18 and over) can open a MyChart account and can allow another adult access to that account. Permanent legal guardians of adult patients can create and access the adult patient’s MyChart account.

There are certain types of health care information that the parent/guardian of a minor patient age 14 or older may not view without the consent of the minor patient. Because of these requirements, MyChart does not contain all of a patient’s health care information. Once your child turns age 14, your child will need to consent to your continued access. If your legal relationship with the patient changes, you agree to immediately inform Children’s by calling 414-266-2640, or sending written notice to Children’s, Health Information Management Department, Suite C140, P.O. Box 1997, Milwaukee, WI 53201, and you agree to immediately stop accessing the patient’s MyChart account.

4. Notification Of New Information in MyChart- You can choose to be automatically notified via e-mail when there is new information to be viewed on MyChart. Your medical information will NOT be sent in this notification. Your employer or anyone with access to your e-mail account may see this notification. You should take this into account when providing an e-mail address.

5. Secure Messaging– You can send secure messages to your provider through MyChart. This type of communication is NOT meant for emergent or urgent situations. If your provider is unavailable, your message may be forwarded to another provider. Please allow about seven (7) business days for a response.

6. Uploading Information- You are able to upload other medical information to MyChart. Please note that uploading information does not make it part of your Children’s health care record and does NOT mean that it will be reviewed by a health care provider. If you have information that must be reviewed by a health care provider, please contact the provider to discuss your concern.

7. Not Medical Information- Using MyChart is not a substitute for obtaining medical care. You should not change or stop any course of treatment without first consulting with your provider. Children’s assumes no responsibility for how you use the information that you access on MyChart.

8. Web site links- MyChart may offer links to related medical web sites not managed by Children’s. These web site link(s) are for your informational purposes only. Children’s is not responsible for, does not endorse and has not verified the accuracy of the information in/on these web sites, and you should not rely on any of the information found on the web sites for purposes of treatment or diagnosis.

9. Privacy- Children’s is committed to protecting the privacy of your health care information. Children’s affords the same degree of privacy to health care information stored on MyChart as is given to health care information stored by Children’s in any other medium. Children’s has taken steps to make MyChart secure against unauthorized access and use. Please see Children’s Notice of Privacy Practices for further information regarding how Children’s may use and protect your health care information.

10. Site Security- For other than general information viewing, MyChart must be accessed with a Secure Sockets Layer (SSL) compatible browser or terminal. Our SSL web server uses authentication and offers encryption technology of 128-bit RC4.

You can tell when you are secure by looking at the location (URL) field. If the URL begins with https:// (instead of http://), the document comes from a secure server. This means your data cannot be read or deciphered by unauthorized individuals. You can tell whether you are truly connected to Children’s by viewing the digital certificate. This certificate verifies the connection between the Children’s server's public key and the server's identification.

11. Availability- There may be times when MyChart is not available due to maintenance or other reasons. Access is not guaranteed at any particular time. If MyChart is not available and you need immediate access to information, please contact your provider.

12. Deactivation- Children’s reserves the right to deny, restrict or revoke access to MyChart at any time for any reason. You may terminate your access at any time by giving written notice to Health Information Management Department, Suite C140, P.O. Box 1997, Milwaukee, WI 53201. It may take 7 days for your request to be processed. For questions regarding terminating access to an account, please contact the Health Information Management Department at 414-266-2640.

I agree to abide by these Terms and Conditions. I understand that I am responsible for my failure to follow these Terms and Conditions. In consideration of my use of MyChart, to the fullest extent permitted by law, I release Children’s Hospital and Health System, Children’s Hospital of Wisconsin, their officers, directors, employees, volunteers and agents from any liability or claim for injury or illness that I or my child may sustain related to using MyChart. I understand that this release applies to myself and my personal representative, heirs, and assigns. I also waive the right I have to bargain for different release of liability terms.
